Services provided at the hospital include emergency medical care, orthopedic surgeries, maternity, and behavioral health services.Visit us online at:  The Transition of Care (TOC) Coordinator is a Registered Nurse, Case Manager who is a healthcare professional that is responsible for coordinating and managing the discharge process for patients in an acute hospital setting.
