Works independently and in consultation with physicians and other health care providers across the health care continuum to ensure appropriate medication therapy for individual patients. Uses patient interviews and counseling, reviews the medical record and medication profile, consults with physicians, teaching, staff education, medication use evaluation, adverse reaction monitoring and resolves medication-related problems. Interventions target high risk population through the provision of medication reconciliation and comprehensive medication use management in focused disease states. Assist with care coordination, disease state education and monitoring to achieve optimal outcomes and is focused on chronic disease state management as part of a coordinated team.Job Description:
EDUCATION:
Doctorate: Graduate of an accredited school of pharmacy.
TYPICAL EXPERIENCE:
5 years recent relevant experience
CERTIFICATION & LICENSURE:
PHARMD-Licensed Pharmacist
OR PHARMR-Registered Pharmacist

Pay Range is $88.28 to $101.51 / hourThe salary range for this role may vary above or below the posted range as determined by location. This range has not been adjusted for any specific geographic differential applicable by area where the position may be filled. Compensation takes into account several factors including but not limited to a candidate’s experience, education, skills, licensure and certifications, department equity, training and organizational needs. Base pay is just one piece of the total rewards program offered by Sutter Health. Eligible roles also qualify for a comprehensive benefits package.
