JOB SUMMARY

* The RN Care Manager is a member of the interdisciplinary group (IDG) who coordinates the patient's care, 24/7 across all settings (home, hospital, nursing home, assisted living, and group homes), to ensure continuous assessment of the patient's and family's physical, psycho-social, spiritual, and end-of-life care needs and implementation of the Hospice's palliative plan of care. Ensures optimal team care coordination to facilitate comfort, meaning, purpose in patient/family end of life closure experiences. All activities will be performed in support of the mission of Phoebe Sumter Hospice.

GENERAL REQUIREMENTS

* Adheres to the hospital and departmental attendance and punctuality guidelines.
* Performs all job responsibilities in alignment with the core values, mission and vision of the organization.
* Performs other duties as required and completes all job functions as per departmental policies and procedures.
* Maintains current knowledge in present areas of responsibility (i.e., self education, attends ongoing educational programs).
