Registered Nurse (RN) - Outpatient Hospice Position Description: Provide Hospice specific nursing care to patients in homes, nursing homes and assisted living facilities Provide initial and ongoing counseling/education regarding illness, disease process, and/or bereavement support Performing comprehensive Nursing Assessment on newly admitted patients Evaluate data collection, formulate a plan of care and establish specific goals for the patient and family/support system Work closely with physicians, communicating pertinent observations Regularly reassess the patient and update the care plan accordingly Coordinate supplies and durable medical equipment, assuring delivery in a timely and cost effective#manner Coordinate inpatient, respite, or acute admissions as well as outpatient services and treatments Serves as patient/family advocate Maintain patient/family dignity # Experience/Qualifications: Current GA RN license Active BLS At least two years of experience, preferably in a hospice setting Extensive travel in service area (must have clean driving record) # Perks/Benefits: (for eligible employees): Have optimal opportunity for career growth within our growing organization Medical / Dental Retirement Plan PTO and paid life insurance## # What Sets Us Apart Archbold Medical Center is a four-hospital, four-nursing-home health system with 540 patient beds.
