As an affiliate of New York Presbyterian Westchester Hospital, Jansen Hospice and Palliative Care you will have the opportunity to work with experienced NYP, Columbia, and Cornell Physicians, Physician Assistants, and Nurse Practitioners as you care for patients throughout Westchester and Bronx County. Recently posted Jansen Hospice patients and NYP employee satisfaction scores are at record highs. This is a community-based public health Nurse Practitioner position. We are seeking someone compassionate and experienced in end-of-life hospice care, with expert knowledge in pain management, Hospice Medicare benefits, hospice diagnoses, and end-of-life disease processes. The Hospice and Palliative Nurse Practitioner assists the Medical Director with the oversight and execution of the medical component of hospice and palliative services. Provide clinical support and education to hospice and palliative multi-disciplinary teams. Collaborates with internal and external providers and referral sources to expand access to services. Serve on agency and hospital-wide committees. The Hospice and Palliative Nurse Practitioner will work within an interdisciplinary team to provide consultation to patients with serious illness who are seeking assistance with advanced symptom management, clarifying goals of care, addressing end-of-life issues and care, and transition planning. Perform outreach activities to grow the palliative program by outreach activities to physicians, hospitals, medical groups, etc., to increase palliative referrals. This is a full-time, M - F 8:30 am - 4:30 pm located at Jansen Hospice and Palliative Care, a Joint Commission Accredited Certified Hospice Agency. Must be willing to rotate when necessary to cover Weekends and Holidays and travel to patient homes throughout Westchester and Bronx counties. Responsibilities of Nurse Practitioners will include, though not be limited to: * Diagnose and treat acute, episodic, or chronic illnesses * Prescribe all necessary medications and treatments, including controlled substances to the extent delegated and licensed and after collaboration with the attending physician and/or hospice physician * Perform comprehensive physical examinations of patients * Develop and implement patient management policies and procedures * Facilitate referrals to other healthcare professionals and medical facilities * Communicate with collaborating physicians or specialists regarding patient care The Hospice Nurse Practitioner provides hospice care through the evaluation and assessment of the health history, health promotion, diagnosis of medical conditions, and management of health problems by directing and developing the plan of care, coordinating, and collaborating with patients, families, and other healthcare professionals.
