Admissions RN Homecare Department: Home Care Position Type: Staff - Non-Exempt Weekends: Flexible $8,000 SIGN ON BONUS Job Summary: The primary function of the Homecare Admission Registered Nurse is to admit new patients to agency services, which include skilled home health, hospice, and palliative care programs. He or she ensures quality and safe delivery of nursing and home health services rendered. As part of the admission process, the Admission Nurse is responsible for accurate and efficient completion of all admission components, including OASIS, HIS and comprehensive assessment and establishment of the initial plan of care in keeping with best practice standards. The Admission Nurse is responsible for completing 2 admissions (or 6 point visit equivalent) per 8-hour day, and may also do revisits as needed to meet agency productivity. Visits completed above 24 visit points in a 32-hour work week are compensated according to the agency's standard per-visit rates. The Admission RN reports directly to the Homecare Clinical Manager. Essential Responsibilities: * Provides skilled nursing care to patients in their homes (including admission/discharge) on a family centered basis while recording/maintaining complete patient record on Portable Electronic Device and/or paper record. * Teaches, demonstrates, supervises and evaluates care given by peers, HHAs, family members and significant others. Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence. * Documents timely and accurate computer entry of patient information to facilitate the development of the OASIS and HIS data set, Enclara Pharmacia, orders for service, creation of time sheets and other required patient/employment documents and reports * According to age and developmental level of the patient, systematically assesses the patient's physical, psychosocial, emotional, cultural, spiritual and discharge needs. .( Agency population 14 y/o to geriatric) * Develops and implements a plan of care for each patient. * Utilizes effective oral and written communication skills to include completeness and accuracy of documentation. * Evaluates the plan of care and reformulates the goals as necessary. * Demonstrates accurate technical skill and operational knowledge of patient care equipment and procedure, including administration of IV medication, TPN, lipids, complex wound care, Pleurex drains etc. Establish the initial contact for patient specific supplies needed for care. (Medications, Wound Supplies, DME, etc. ) * For Hospice patient's ; recognizes symptoms requiring intervention and initiates these at the SOC as deemed appropriate. * Recognizes unsafe and life‑threatening situations and intervenes appropriately. * Participates in the agency's continuous quality improvement program. * Can be depended upon to report to work on-time, use time off appropriately, and complete annual education & medical requirements. QUALIFICATIONS: Education: * Graduate from an accredited School of Nursing required. Experience: * Two years of recent medical/acute care or critical care experience as a registered nurse, or an equivalent level of experience preferred. Achieve Home Health Care specific competencies within the timeframe determined by the department. * Current license to perform as a registered nurse in the State of Connecticut required.. * Valid Connecticut Driver's License and proof of current insurance coverage required.
