Job Summary: You would be joining the Ayer Neuroscience Institute at Hartford Hospital in the position of Stroke Coordinator. This role has the benefits of being part of the Institute along with the resources and teamwork that it brings. Your role, in partnership with the interdisciplinary team, would be to support, manage and coordinate systems related to acute stroke care delivery at Hartford Hospital, optimizing the care and access. In conjunction with the Manager of the Stroke Program at Hartford Hospital, the Stroke Coordinator is responsible for the development, implementation, coordination and evaluation of the stroke program as well as the management, education and current evidence-based practice related to stroke care and prevention. In addition, the coordinator adheres to the clinical practice guidelines set forth by the Medical Director and the Joint Commission standards for quality care. The coordinator also provides support for the stroke program to medical and nursing staff and those departments, which have direct and indirect patient care responsibilities. The Stroke Coordinator supports patient, assisting them in the navigation through the care process. Lastly, the stroke coordinator is responsible for problem identification, problem solving, education, research, and performance improvement including QA/QI initiatives for the program. Job Responsibilities: * Provides leadership for the development and maintenance of guidelines and order sets for continuing care of stroke patients to ensure best practices are followed. * Serves as a resource and collaborates with members of the community and interdisciplinary team to serve as a resource to individuals providing care to stroke patients. * Plans and coordinates initial and ongoing training/education for the multidisciplinary team. * Collaborates with the EMS coordinator to deliver education to local EMS providers in order to maintain high levels of pre-hospital care. * Provides Education to Patients and assists them in navigating through the levels of care from inpatient to outpatient * Guides, supports and executes ongoing performance improvement projects related to improving care and outcomes of stroke patients. * Utilizes data to support ongoing initiatives. * Maintains all pertinent data related to stroke program activities. * Tracks and reports on specific performance and outcome measures. * Monitors performance indicators, quality of care issues, cost of care and guidelines for stroke care management. * Reviews Stroke Patient care daily at Hartford Hospital. Works with Nursing staff to ensure TJC standards are met and patients are receiving appropriate and efficient care. * Participates in continuing education opportunities such as stroke conferences and nursing education for stroke care. * Assists in Preparing annual report to Joint Commission. * Acts as clinical liaison between physicians, clinical coordinators and administrative staff. * Perform other related duties as required Qualifications * Bachelor's in Nursing or related field, Master's Degree in Nursing preferred. * At least 5 years of recent acute care clinical experience * Strong clinical background in Medicine and Neurology * State of CT RN license or eligibility to do so prior to date of hire * BLS, ACLS, NIHHS certifications * Certification in stroke or neuroscience We take great care of careers.
