How you'll help Are you a pharmacist who wants an opportunity to work directly with patients in a provider's office? Have you thought about how much impact you could make working one-on-one with patients in their pharmaceutical care? Have you worked doing medication therapy management in the community setting and want to take it to the next level to proactively make changes with the provider in the office? In this unique role, you'll work collaboratively and practice on-site with Primary Care physicians and their practices. The pharmacist will collaborate with the patient's primary care provider to optimize evidence-based pharmaceutical care, develop appropriate care plans, and follow-up with patients based on individualized patient needs. This is a unique and excellent opportunity to provide patient care and to be able to shape the future of ambulatory clinical pharmacy. What you'll do * Evaluate, monitor, recommend and design patient-specific pharmacotherapeutic regimens * Participate in pharmacy activities to assure safe, effective, appropriate, and economical use of medications to improve continuity of care and outcomes * Be responsible for managing chronic conditions (diabetes, hypertension, COPD, etc.) and getting these patients to goal * Help prevent avoidable medication-related hospitalizations * Serve as a resource to practice physicians, nurses, and other team members on the general drug information * Regularly monitor patient data and metrics to evaluate program effectiveness and clinical outcomes * Understand the medication-based quality measures as used in Pay for Performance, Star, and HEDIS measures * Document clinical activities, outcomes, and quality intervention results * Provide education on prescribed medications and compounds according to professional standards and practices * Participate in committees and meetings to facilitate the improvement of patient care and the development of goals, programs, and objectives for specialized patient populations * Participate in problem-solving where appropriate about best practice development for medications such as high cost, or otherwise sub-optimal therapies * Accept and perform other duties as assigned Experience and skills you'll need * Currently hold a Board of Pharmaceutical Specialty Certification or have completed an ASHP Residency with two years of clinical experience OR ... * Have a Doctor of Pharmacy Degree with three years of clinical experience plus a certificate program * PharmD Degree required * BCPS certification preferred * Current active license to practice pharmacy in the state in which you are applying required * National Provider Identifier (NPI) required * Extensive knowledge of pharmacy and its related subdisciplines (therapeutics, pharmacology, physical pharmacy), formulary development, drug use review, quality assurance, legal, regulatory, and standards of pharmacy practice * Ability to integrate with the healthcare team * Experience in the medication reconciliation process * Interdisciplinary department support for education and monitoring patient drug therapy * Proficient at working with or learning how to work with Electronic Health Records Systems * Working knowledge of the use of enterprise data warehouses and patient registries * Computer skills including Microsoft Teams, Word, Office, Excel, and PowerPoint * Works independently to meet expected deadlines and deliver excellent patient care * Understanding of the principles of clinical integration and population health management * Ability to work collaboratively as a member of an interdisciplinary team, establish and maintain effective working relationships with other healthcare professionals, practitioners, patients, and families Must live in a commutable distance to Greenville, NC - this role is performed in provider clinics and will require regional travel to a group of clinics in and around Greenville, NC.
