Case Management Team Lead- Care Coordination


Job Summary Under general supervision of the Manager, the Team Lead will support the clinical expertise of the Case Management Department. Accountable for coordinating departmental activities including daily staffing, scheduling, and process workflow to ensure team members follow established policies, procedures, and professional guidelines. Arranges for staff coverage when indicated and provides coverage as necessary to patient needs. Provides side-by-side support, mentorship, and skill development to all team members. The team lead promotes collaboration among departmental disciplines and interdisciplinary partners. Identifies strengths and opportunities in clinical practice, capturing themes for process improvement to escalate to leadership. Strong problem-solving required for effective real-time intervention. Responsible for ensuring that all staff perform job functions in accordance with mission, vision, and values of Tampa General Hospital. n this role candidate will also be responsible for Case Management role development in the following areas: Emergency Department, Observation, Utilization, Pre-Admission Testing, Pediatrics, Mom-Baby, NICU, and Case Management Extenders. * Experience: Three (3) years clinical experience with one (1) year of experience in Case Management or two years in Emergency Management. * Education: Graduate of an accredited school of Nursing. Associate degree required. BSN preferred. * Licensed to practice as a Registered Nurse by the State of Florida * Ability to read, write and speak English clearly and distinctly. Knowledge of Microsoft Office including Word, Excel, and PowerPoint required. Knowledge of Microsoft Access, Project, Publisher, and MS Vizio, Microsoft Team Presenting platform, audio/visual hardware desired.

Pay

Average Florida Staff Position Pay

$39.48/hour
