Responsibilities Our Comfort Care RNs provide direct patient-centered hospice services at the bedside of our enrolled participants while coordinating physical, emotional, and spiritual support through our interdisciplinary team. Support the provision of high-quality care for end of life (EOL) focused on minimizing symptoms, pain, and providing psychosocial support during the transition. Offer professional triaging, nursing care and support to educate family members about the process, how to manage and control symptoms, and offer support and strength during difficult times. Our Comfort Care RN is there to be the main point of contact for the participant and their family through the transition. Working with a high level of autonomy our nursing team needs to exercise critical thinking skills, triage family needs and work back through the IDT with excellent communication skills. Participant Care * Triage participant and staff phone calls to determine priority of care and routes accordingly. * Implement orders written by primary care physician(s). * Instruct participants and family regarding medications and treatment instructions. * Administer and monitor medications, provide training in self-administration, and medication compliance. * Assist in the examination, assessment, evaluation testing, and treatment of participants while observing, recording, and reporting on participant's current condition and needs. * Perform initial End of Life assessments on participants to determine individual needs and coordinate a Plan of Care with care-giving resources. End of Life Support * Develop EOL documentation, forms, standards, protocols, and overall philosophies for the scope of the position. * Conduct initial EOL assessments of participants to determine their individual needs and coordinate a plan of care. * Provide decision-makers with care options and resources to aid them in making difficult decisions in the best interest of the participant. * Educate family members about the end-of-life process and ensure they receive the "Caregiving at Life's End" manual. Documentation and Collaboration * Maintain participants' medical records and fulfill agency charting and reporting requirements in a timely manner. * Serve as a member of the Multi-disciplinary Team, attending meetings and collaborating on care planning. * Participate in developing, organizing, and presenting staff in-services and working with in-home service techs as needed for specific care techniques. * Provide psychosocial support to participants and their families, including coping and bereavement resources in collaboration with the Social Work department. * Inquire about participants' and families' spiritual needs and call upon appropriate resources. * Participate in an interdisciplinary team to formulate Plans of Care for InnovAge PACE program participants! * Additional work functions and duties may be assigned. REQUIRED * Current State issued Registered Nurses License * Current CPR certification or the ability to obtain CPR certification * 2+ yrs health care with an emphasis in geriatrics, with at least one year being an EOL/Hospice. * Experience and knowledge of supportive/bereavement counseling techniques, strategies and available resources and the ability to apply/inform family members and/or caregivers. * Experience with conducting end of life assessments and developing appropriate plans of care (POC). * Require personal transportation, current state issued driver's license, good driving record and auto insurance * Associate's degree in nursing PREFERRED * Bachelor's degree in nursing * Certification as a Gerontological Nurse * 3+ yrs health care experience with emphasis in geriatrics and broad knowledge of health care problems that afflict the frail/elderly. * Bi-lingual - Spanish Benefits The mission of InnovAge is to allow seniors to live life on their terms - by aging in place, in their own homes and communities, for as long as safely possible. Salary ranges are dependent on a variety of factors, including qualifications, experience, and geographic location and does not include potential bonus or benefits. Posted Salary Range USD $45.75 - USD $50.00 /Hr.
