This position reports to the VP Care Coordination and will provide supportive care to the patient and family after normal business hours on an on-call basis. This position will also manage and direct an interdisciplinary group consisting of Registered Nurses, Licensed Practical/Vocational Nurses, Hospice Aides and support staff to enhance the quality of hospice care. * Maintains an accurate and updated medical record, including all Interdisciplinary notes, Plans of Care, Initial and on-going assessments and any other information pertinent to the care of the patient. * Functions as the Administrator on-call throughout the weekend hours * Performs the initial and on-going assessment of the patient as needed * Helps develop and implement the Hospice Plan of Care as prescribed by the physician and in conjunction with the other members of the Interdisciplinary Group About You * Registered Nurse with minimum of Associate's Degree in Nursing * Bachelor's degree in Nursing (BSN) from an accredited school of nursing preferred * Current license to practice nursing in the state of work * Minimum of 1 year experience in hospice * Minimum of 1 year of management experience preferred We Offer * Comprehensive Benefits Package: Health Insurance, 401k Plan, Tuition Reimbursement, PTO * Opportunity to participate in a Fleet Program * Competitive Salaries * Mileage Reimbursement * Professional growth and development opportunities Legalese * This is a safety-sensitive position * Employee must meet minimum requirements to be eligible for benefits * Where applicable, employee must meet state specific requirements * We are proud to be an EEO employer * We maintain a drug-free workplace Location Gentiva
