Exciting Opportunity: Registered Nurse for Forensic Assertive Community Treatment (FACT) Program The schedule for this position Monday through Friday, daytime business hours. This position is community based. Medical supply and equipment is mobile to support community-based model. Program Description: Our Forensic Assertive Community Treatment (FACT) Program is a beacon of hope for individuals navigating complex mental health challenges and justice involvement. Through a multidisciplinary and community-based approach, FACT offers intensive, time-unlimited services aimed at improving mental health outcomes, daily functioning, and reducing recidivism. From individual therapy and case management to crisis support and medical coordination, FACT provides a holistic framework for healing and empowerment. Position Overview: As a Registered Nurse for our FACT Program, you'll play a pivotal role in coordinating a wide range of supportive and therapeutic behavioral health services for adults with complex needs. From medication management to crisis intervention, you'll provide compassionate care while supporting a trauma-informed environment. With an understanding of the effects of trauma on health and coping, you'll empower our clients on their journey toward recovery and wellness. Key Responsibilities: * Provide quality nursing care, including medical interventions and documentation. * Formulate Nursing Care Plans for clients' medical/physical conditions and administer medications as prescribed. * Collaborate with the interdisciplinary team to develop and implement treatment plans. * Act as a liaison with medical staff, pharmacy, and other outside agencies to ensure continuity of care. Acts as the face of medical care for FACT team and take the lead on all medical case management and coordination. * Administer psychotropic medications and interpret client response to medication. * Document client contacts, progress, and relevant events in compliance with regulations. * Assist in crisis situations, following emergency protocols and trauma-informed interventions. * Educate clients about medications, medical treatments, and ongoing support strategies. * Engage in health promotion, prevention, and education activities within the community. * Provide on-call crisis response and back-up interventions as needed. Qualifications: * Active Oregon Registered Nurse License and CPR/First Aid Certification. * QMHP or QMHA credential as outlined in OAR 309-016-0005. * Minimum two years of clinical experience in inpatient or outpatient settings preferred. * Knowledge of medical conditions commonly encountered by individuals with severe mental illnesses. * Motivational Interviewing skills and ability to maintain a trauma-informed care environment. * Proficiency in computer hardware and software for job functions. * Valid driver's license, clean driving record, and ability to pass Defensive Driving course.
