UAS RN Assessor

UAS RN Assessor COME WORK FOR THE LEADING, LOCAL MANAGED CARE COMPANY - VILLAGE CARE! VillageCare is looking for self-motivated, passionate RN's for a Full Time Nurse Assessor. The Nurse Assessor conducts comprehensive assessments of all aspects of potential new members' condition - clinical, social and environmental to determine the level of services needed to appropriately keep member safe in the community. Some daily activities will include: * Utilize clinical skills to assess and document all aspects of the potential members long-term community based needs. * Provide community-based training and education for frail population care. * Conduct face to face UAS-NY assessments according to state guidelines, policies, procedures, and protocols. * Gather clinical information from various sources to address all conditions. * Communicate with consumers, families, providers, and other parties as needed to complete accurate comprehensive assessment. * Explain VillageCareMax features and benefits, including explanation of the member's handbook. * Consult with supervisor and others in overcoming barriers in meeting goals and objectives. We would like to speak to those who have a current unrestricted NYS RN license along with 1+ year home care, hospice, or field based case management experience preferred. Proficiency in use of UAS-NY required. Prior MLTC/ Managed Medicaid/MAP/FIDA experience preferred. In home visits required - up to 5 days. There are many benefits to working for VillageCare.
