Case Manager Registered Nurse (RN), Case Management

Job Overview: The Case Coordinator works collaboratively with patients, physicians, family and providers of healthcare to implement a plan of care which meets the individual s needs. This is coordinated by assessing, planning, implementing, advocating, monitoring, and evaluating options and services, to meet the individuals health care needs. Qualifications: Required: * Must be a graduate from an accredited school of nursing * 5 years acute hospital experience * Additional experience required based upon specific department needs Preferred: * BA/BS/BSN Degree in Nursing * Completion of Clinical experience in NLN accredited school Certifications and Licenses Required: * Current Registered Nurse License in the state of New Jersey * Basic Life Support (BLS) from the American Heart Association * National certification preferred Scheduling Requirements: * Full-Time * Day shift * 40 hours weekly Other Duties: Please note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
