The primary role of the Nurse Navigator is to: * function as a health advocate to aid individuals in receiving the physical health and behavioral health care * assist in coordinating medical and behavioral health appointments, hospital care and discharge to the community * arranging travel to appointments * development of personal plans for consumer health collaboration and wellness.
