The schedule for this position is Wednesday through Friday - 8:30 a.m. to 5:00 p.m. Cascadia's primary care programs offer staff a healthyand sustainable workload, seeing a total of 12 to 18 clients each day with plenty of time to complete other daily tasks. Appointments are typically 20 minutes for established clientsand 40 minutes for new clients, emphasizing a balanced pace for the MA's daily responsibilities. The Registered Nurse for Primary Care is responsible to assist the primary care providers in all aspects of triage and patient care management. The RN assumes responsibility for treatment decisions in keeping with RN licensure provisions and renders care prescribed by physicians and nurse practitioners, while supporting a Trauma Informed Care environment. The RN-PC supports the expansion of integrated health care throughout Cascadia's services through health education and clinical support to the Care Coordinators. The RN-PC works with Cascadia's Director of Nursing, the Primary Care Medical Director and clinical leadership to train and lead the integration of primary care skills and practice into the portfolios of other nursing staff throughout Cascadia's services. The RN-PC works with clinic leadership to support and often lead improvements in quality of care, patient health outcomes and productivity. All incumbents must understand, appreciate and respect the diversity and cultural differences within our Cascadia community. It is expected that there is an awareness and sensitivity to the perspectives and perceptions to those we serve, as well as our colleagues. As Cascadia is Trauma informed, the incumbent must support a Trauma Informed Care Environment. With the high prevalence of trauma among individuals receiving behavioral health services, it is required that the incumbent understand the effects of trauma on health, coping, and other aspects of the lives of those we serve. Essential Duties This position description is not intended to be an all-inclusive list of responsibilities or an all inclusive job description. Clinical * Perform an intake assessment which includes obtaining and reviewing prior medical records and medical or program-related financial data, documenting a complete medical history, assessing cognitive/verbal skills and needs and identifying barriers to accessing healthcare. * Under standing orders, accomplish point of care laboratory testing, immunizations, TB skin testing and referrals for preventive health needs as indicated per disease management protocols. * Identify patient symptoms, signs and disabilities and document on patient record. May check vital signs and other physical findings and record on patient record. * Monitor patients frequently for changes in health status after initiation of a new medication, a hospitalization or recent decline in function. * Follow-up with patients when barriers to referrals are identified. * Triage and facilitate response to urgent telephone calls, requests or visits from assigned patients. * Assist with arranging supportive services as needed. * Provide counseling and facilitate screening for HIV, other STDs, and Hepatitis as indicated. * Monitor lifestyle factors affecting health such as tobacco use, substance abuse, nutrition and physical activity and assist the patient with goal-setting to achieve behavioral change. * Participate in regular staffing meetings focused on coordinating patient care within an interdisciplinary team, keeping the team updated on patients' conditions and circumstances. Educational * Perform individualized assessment of patients' educational needs and provide tools to aid patients in managing their disease(s) effectively. * Provide individual and family educational interventions including self-management goal-setting, counseling and training on the habits, lifestyle changes, supplies and tools necessary to manage their disease. * Provide individual counseling on office procedures, eligibility for programs/services, importance of a primary care medical home and other health issues. * Teach group classes covering topics which build skills in self-management of one or more chronic diseases. * Assist in the development and maintenance of a library of educational resources including written materials and videos, on related health issues. Serve as a consultant to the rest of the health care team for educational resources, reviewing them for language, cultural competency and reading level. * Assist in gathering and compiling data to direct the most effective use of educational interventions. * Follow-up with patients to assess whether educational objectives are met and determine need for refresher interventions. * Provide individual and family educational interventions including self-management goal-setting, counseling and training on the habits, lifestyle changes, supplies and tools necessary to manage their disease. * Provide individual counseling on office procedures, eligibility for programs/services, importance of a primary care medical home and other health issues. * Teach group classes covering topics which build skills in self-management of one or more chronic diseases. * Assist in the development and maintenance of a library of educational resources including written materials and videos, on related health issues.
