Job Description:
Supplemental Health Care is seeking Social Workers in Madison, Tennessee with prior OASIS/Medicare Certified Visits for contract assignments with a partnering home health agency.
Home Health Social Worker Contract Details:
- $1,700 - $1,881 per week
- SHC Home Health Social Workers are paid by the hour, not by the visit
- Travel and mileage reimbursement available on all contracts
- Social Worker will have a designated territory of coverage
- Weekly expectation is 30 units of productivity
- Home Health contracts with SHC require previous experience with OASIS/Medicare Certified Visits

Requirements:
- OASIS/Medicare Certified Visits Experience
- Electronic Charting experience
- Minimum of 1 year of recent home health Social Worker experience (private duty does not qualify)
- Most contracts require some on-call
- Current Tennessee Social Worker License
- American Heart Association BLS
- Valid U.S. Driver's License
